How can a patient who had a full / radical prostatectomy years ago, show a high PSA level again?

Well, we know that PSA (Prostate Specific Antigen) is produced by cells of the prostate gland (the epithelium) and it is reasonable to assume that cells continue to produce PSA, even if they are in places outside the prostate? That which makes a high level of PSA in a prostate cancer patient in remission and full / radical prostatectomy indicates the presence of metastatic tumor somewhere? If not how and why it might PSA levels be so high in the patient described above? And now he and his wife are worried to pieces with his most recent elevated PSA results.

Because PSA is prostate specific, I'd be very concerned about the recurrence or metastasis. He could have been of tumor invasion was lost, or later, the previously undetected metastases at the time of surgery. My best to your friend … good luck with a malignant disease.
